I, understand that participation in off-site activities organized by White Oak Spring Baptist Church ("the Church") involves certain risks specific to the location and nature of the activity described above. By signing this waiver, I acknowledge these risks and agree to the following: ACTIVITY RISKS: I understand that this activity may involve specific risks including but not limited to: [physical injury, death, emotional injury, property damage or other losses]. VOLUNTARY PARTICIPATION: I voluntarily choose to allow the above participant to engage and participate in this activity and assume all risks associated with this participation. WAIVER AND RELEASE: To the fullest extent permitted by law, I release the White Oak Spring Baptist Church (“the Church”), its leadership, employees, volunteers, and representatives from all liability, claims, demands, or causes of action for injury, death, property damage, or loss arising from my participation in this activity/event.
