I, as a volunteer youth leader with White Oak Spring Baptist Church ("the Church"), understand and acknowledge the responsibilities and risks associated with youth ministry. By signing this document, I agree to the following: VOLUNTEER STATUS: I acknowledge that I am serving as a volunteer and not as an employee of the Church. I am not entitled to any compensation or employee benefits. ADHERENCE TO POLICIES: I have received, read, and agree to follow all Church policies regarding youth ministry, including but not limited to: child protection policies, transportation guidelines, supervision requirements, and reporting procedures. BACKGROUND CHECK CONSENT: I authorize the Church to conduct appropriate background checks and reference verifications. I affirm that I have disclosed all criminal history and understand that certain offenses may disqualify me from youth ministry. APPROPRIATE CONDUCT: I commit to maintaining appropriate boundaries and relationships with youth under my care, following the two-adult rule, and adhering to all safety protocols established by the Church. TRAINING REQUIREMENTS: I will complete all required training programs and attend regular leadership meetings as scheduled. ASSUMPTION OF RISK: I understand that youth ministry activities involve inherent risks. I voluntarily assume these risks and will exercise reasonable care in all activities. WAIVER AND RELEASE: To the fullest extent permitted by law, I release White Oak Spring Baptist Church, its leadership, employees, and other volunteers from all liability, claims, demands, or causes of action for injury, property damage, or loss arising from my volunteer service, whether caused by negligence or otherwise. REPORTING OBLIGATION: I understand my legal obligation to report suspected child abuse or neglect to appropriate authorities and will follow Church reporting protocols. CONFIDENTIALITY: I will maintain appropriate confidentiality regarding sensitive information about youth and families, while fulfilling all mandatory reporting obligations. DISPUTE RESOLUTION: I agree to attempt to resolve any disputes with the Church through Christian mediation before pursuing legal action. This acknowledgment and waiver shall remain effective until my volunteer service ends or is terminated. I have read and understood this document and signed it voluntarily.
